Sanjivan Kohli, MD

Dr. Sanjivan Kohli is the Medical Director of Be Well Collaborative Care, who oversees the quality assurance aspect of patient care and ensures the successful synergy of our multidisciplinary healthcare providers.
Dr. Sanjivan Kohli is a pulmonologist, who is currently affiliated with multiple hospitals in the Orange County area, including Long Beach Memorial Medical Center and Mission Hospital. Dr. Kohli received his medical degree from the Royal College of Surgeons in Ireland. Dr. Kohli currently specializes in Internal Medicine and Pulmonary Critical care.
Soroush Zaghi, MD
Otolaryngology & Sleep
Dr. Zaghi graduated from Harvard Medical School and completed a 5-year residency training in Head and Neck Surgery at UCLA. He completed Sleep Surgery Fellowship as Clinical Instructor of Otolaryngology at Stanford University. The focus of his specialty training is on Sleep Endoscopy, CPAP Optimization, Myofunctional Therapy, Frenuloplasty, Nasal Surgery, Throat Surgery, and Maxillofacial Surgery for the treatment of nasal obstruction, snoring, upper airway resistance syndrome, and obstructive sleep apnea. He is very active in clinical research relating to sleep disordered breathing with over 50 peer-reviewed journal articles relating to neuroscience, head and neck surgery, and obstructive sleep apnea.
Gary Feldman, MD
Pediatric Sleep Medicine
Dr. Feldman received his bachelor of medicine and surgery degree from the University of Cape Town School of Medicine in South Africa. He completed his residency in pediatrics at Red Cross Children's Hospital and University of Cape Town in South Africa and worked in primary care pediatrics for five years prior to moving to the United States in 1999. He completed a fellowship in developmental and behavioral pediatrics and simultaneously trained in sleep medicine at Rainbow Babies and Children's Hospital and Case Western University in Cleveland, Ohio. He has 16 years of experience treating sleep disorders in children, especially those with special needs such as autism, developmental delays and genetic disorders. Dr. Feldman is able to manage the behavioral, medical, and breathing related aspects of sleep disorders. His treatment paradigm is to provide big-picture solutions for children with sleep conditions. One of his catch phrases is, “children who don’t sleep well, don’t behave and learn well. Their sleep difficulties have a direct impact on the quality of life of other family members”. He is of the opinion that for appropriate and definitive treatment, a multidisciplinary approach is often necessary.
Andrew Lee, MD
Dr. Lee completed his undergraduate studies at Stanford University. At Stanford, he obtained his degree in Computer Science with a minor in Biology (Class of 2000). Afterwards, he spent half a year volunteering at various community outreach clinics throughout Guatemala, and learning Spanish. He then obtained his medical degree at Washington University in St. Louis, one of the top-ranked medical schools in the U.S. Dr. Lee completed internship and residency training at Children’s Hospital of Orange County (CHOC), and received the prestigious Merle J. Carson (“Resident of the Year”) Award for Excellence at his graduation.
Dr. Lee believes that the best medical care for children comes from a personalized approach, where families and children actually get to know their pediatrician, have access to the doctor, and see the same doctor as they grow up. Within the medical field, Dr. Lee has a special interest in treating children with asthma and eczema, and has also undergone extensive additional training to treat children with attention problems (ADHD) and mood/behavior disorders.
Deepak Shrivastava, MD

Dr. Deepak Shrivastava received his Sleep Medicine education and training from Stanford University. Dr. Shrivastava is also board certified in Sleep Medicine, Critical Care Medicine, Pulmonary Medicine, Internal Medicine, and Polysomnographic Technology. Currently, Dr. Shrivastava serves as Chief of Sleep, Critical Care and Pulmonary Services at San Joaquin General Hospital. Additionally, Dr. Shrivastava holds two academic positions. He is currently a Professor of Internal Medicine, Sleep Medicine, Pulmonary and Critical Care at the University of California – Davis School of Medicine. Dr. Shrivastava is also an Assistant Professor of Medicine at the School of Osteopathic Medicine in Vallejo, California. Dr. Shrivastava is continuously and directly involved in healthcare quality and performance improvement activities.
Karen Cheng, MD

Dr. Karen Cheng is a board certified physician, whose areas of expertise include Sleep Medicine & Neurology. Dr. Cheng is an active member of the American Academy of Sleep Medicine, the American Academy of Neurology, and the California Sleep Society. Dr. Karen Cheng received her Bachelor's degree from Stanford University. After graduating with honors from Stanford University, Dr. Cheng received her medical degree from the Medical College of Virginia. Dr. Cheng's residency and internship were completed at the University of California – Irvine. Dr. Cheng's areas of expertise are sleep disorders, such as obstructive sleep apnea, insomnia, narcolepsy, restless leg syndrome, and REM-sleep behavior disorder. Dr. Cheng currently provides both pediatric and adult sleep medicine consultations and also reviews and interprets polysonogram studies.
Lenore Morrissey, MPT, OCS, CFC

Lenore Filler Morrissey was born and raised in the Los Angeles area. She earned her undergraduate Bachelor of Arts degree in Biology at Whittier College in 1993. She continued her studies in sciences at San Francisco State, Long Beach State and Louisiana State Universities. Lenore was accepted into the Feinberg School of Medicine Program in Physical Therapy at Northwestern University and graduated with her Masters of Physical Therapy Degree in 1998. Upon returning to California, she worked in a variety of physical therapy settings including pediatrics, home health and inpatient rehabilitation.
While working at Kaiser Permanente in both in and outpatient settings, she was trained in manual physical therapy and achieved her Orthopedic Certified Specialty from the American Board of Physical Therapy Specialists 2004. Then in 2005, Lenore opened and operated a private outpatient orthopedic physical therapy practice in Rancho Palos Verdes for 11 years. While running her practice, she saw a growing need for expertise in head and neck disorders and airway dysfunction. In 2012 she studied oral myofunctional therapy under Joy Moeller, BS, RDH a leading international instructor in this field. She has recently earned her Certification as a Craniofacial Specialist under the training of renowned expert in craniofacial and temporomandibular disorders, Mariano Rocabado via the University of St. Augustine. She is currently finishing her transitional doctorate in physical therapy with this university. Lenore is a member of the faculty for the Airway Focused Dentistry Mini-Residency. She continues study in craniosacral therapy, myofunctional therapy, capnometry and upper airway disorders to bring an integrated knowledge that is unique in physical therapy. She treats private patients both in Los Angeles and Orange County. Her mantra; “Total body well-being begins with a patent airway in concert with proper alignment and function of the structures of the head, neck and tongue."
Susan Sklar, MD
Dr. Susan Sklar is a Harvard trained gynecologist, who received her Bachelor of Arts degree from the University of Chicago. Dr. Sklar received her medical doctorate degree from the Boston University School of Medicine, with a completed internship at the Boston City Hospital. Dr. Sklar's credentials also include a residency at the Boston City Hospital and a subsequent residency at the Waltham Hospital. Most notably, Dr. Sklar completed an additional 3 year residency at the Beth Israel Hospital, Harvard University Affiliate. Some of Dr. Sklar's affiliations, honors, and awards include an ongoing fellowship at the American College of Obstetrics and Gynecology. Additionally, Dr. Sklar held a visiting lecturer position at the University of Southern California-Department of Family Medicine (2002 - 2004) and an associate clinical professor position at the University of California, Irvine-Department of Obstetrics and Gynecology (1983 - 2003).
Mike Dubik, MD

Dr. Mike Dubik is a board certified physician, whose areas of expertise include Sleep Medicine and Pediatrics. Dr. Dubik's medical career began in the U.S. Navy. While in the Navy, Dr. Dubik completed two tours as a flight surgeon, worked as a staff pediatrician and was appointed the Chair of Pediatrics at Navy Medical Center in San Diego. Dr. Dubik completed his Sleep Medicine Fellowship at Stanford University, and is currently a member of the American Academy of Sleep Medicine, the Society of Behavioral Sleep Medicine and the American Academy of Pediatrics.

Mark A. Cruz, DDS
Mark A Cruz graduated from the UCLA School of Dentistry in 1986 and started a dental practice in Monarch Beach, CA upon graduation. He has lectured nationally and internationally and is a member of various dental organizations. He was a part-time lecturer at UCLA and member of the faculty group practice and was past assistant director of the UCLA Center for Esthetic Dentistry. He has served on the National institute of Health/NIDCR (National Institute of Dental & Craniofacial Research) Grant review Committee in Washington D.C. as well as on the editorial board for the Journal of Evidence Based Dental Practice (Elsevier) and is currently serving on the DSMB (data safety management board) for the NPBRN (national practice-based research network) .Dr. Cruz is the Chief Executive Officer of Be Well Collaborative Care and is currently one of the most prominent voices in the field of Airway Focused Wellness Solutions.
Ellen Crean, DDS, PhD
As a partner at the Craniofacial Pain and Sleep Center of Arizona, Dr. Crean spent 10 years working with craniofacial pain, TMD, and sleep patients. Dr. Crean is the co-founder and Chief Operating Officer of Be Well Collaborative Care. The doctor works closely with the physicians, sleep techs, respiratory therapists and the sleep labs, to ensure that optimum clinical outcomes are achieved. She is also a partner in Kosmo Technologies, Per4maxPlus, LLC and is a consultant to Triton Medical Solutions. Dr. Crean has taught nationally on the topic of oral appliance therapy for sleep disordered breathing, addressing the business, insurance, and clinical aspects of the treatment. She is a member of the Academy of Clinical Sleep Disorders Disciplines, American Academy of Sleep Medicine, American Academy of Dental Sleep Medicine, and the American Academy of Craniofacial Pain.
